Welcome to the Children & Young People’s Transformation Plan ‘Refresh’ – Survey


Who are Healthy Young Minds?

We are a group of 25 (or so) individuals who work in the NHS, social care, schools, charities, community teams and hospitals. We are officially referred to as the Children & Young People’s Emotional Mental Health and Wellbeing Transformation Board. We are dedicated to working together with young people and their families to change mental health and wellbeing services in Lancashire for the better, giving our children and young people the best start in life. In March 2015 we developed a five year plan, called the Transformation Plan, to help us to do this. In 2016 we refreshed our plan and used feedback from the survey to inform the refresh.

Why do we need your help?

We are now in our second year of transformation and have achieved lots of good things, but it is now time to look back and reflect on the work we have been undertaking and also consider what we now need to do going forward.  In order to do this we have ‘refreshed’ our plan and we would like to know what you think about it!  If you would like to take part in a short survey please follow the link below and leave your feedback.
