To deliver our 2030 Vision for Health and Care in Fylde and Wyre ( pdf | 625.7 KB ), GPs across Fylde and Wyre have come together in geographical ‘neighbourhoods’ to adapt services to meet the needs of local people.


Each of the four neighbourhoods has developed its own priorities based on the experience of the GPs within that area. Work is currently underway to make these priorities a reality.

  • Fleetwood – A home visiting team for patients with long-term conditions and significant unplanned hospital attendances
  • Kirkham and Wesham – Episodic care and the development of a primary care same day access service plus community nursing and the co-ordination of long-term condition care
  • Lytham, St Annes, Freckleton and Ansdell – Structured chronic disease monitoring and management for patients who are housebound or in care homes
  • Poulton, Thornton and Over Wyre – Care home service based on a successful pilot previously undertaken
