NHS Fylde and Wyre CCG is committed to enhancing the quality of care within care homes by supporting staff and managers to deliver the best possible care.


Fylde and Wyre Interim Care Home Service

A new interim care home service is up and running in Fylde and Wyre which offers support to all four neighbourhoods.

The service, which is provided by Blackpool Teaching Hospitals Community Services, started on 1 November 2017 and will run until 31 March 2019. It provides a whole system approach to all 69 care homes in Fylde and Wyre and works across two hubs:

  • Fylde team – based at Ansdell Medical Centre (Kirkham and Wesham and Lytham, St Annes, Ansdell and Freckleton neighbourhoods).
  • Wyre team – based at Thornton Medical Centre (Fleetwood and WIN neighbourhoods).
The CCG’s care home offer aims to strengthen support to care home residents (nursing and residential) and care home staff, to ensure they have direct access to clinical advice, including appropriate on-site assessment via a proactive and preventative care service.

The interim service offer works with primary care services, Enhanced Primary Care, the falls service and community services while the future neighbourhood care teams become established in the new ways of working to meet the needs of its neighbourhood population.

From April 2019 the care home service offer will then be provided by the neighbourhood care teams.

The service has been designed to improve proactive and reactive primary care services for all care home residents based on a “person-centred” care plan, with coordinated input from care home staff, general practice and multi-disciplinary teams (MDTs) provided through community services, mental health service and social care.

Care home interventions:

  • New residents will be clinically triaged within two working days.
  • New residents will be seen for a first holistic assessment within 14 days of registration.
  • Facilitate advance care planning discussions with care home staff and residents, ideally within six weeks.
  • Monthly care home ward rounds to discuss residents’ needs and where appropriate via MDT supported by Care Home Connect.
  • Post discharge from A&E or hospital reviews within 72 hours.
  • Medication reviews and pharmacy optimisation.
  • Dementia screening.
