Please see this announcement by NHS England which sees the Fylde Coast chosen as one of 29 ‘Vanguard’ sites to take a national lead on transforming care for patients.

This announcement that the Fylde Coast has been chosen to test the ‘multispecialty community provider organisational model’ identified in NHS England’s Five Year Forward View, shows that excellent progress is being made in joining up health and social care for the benefit of patients.

A spokesperson for NHS Blackpool CCG, NHS Fylde and Wyre CCG, and NHS Blackpool Hospitals NHS FoundationTrust, said:

“We are absolutely delighted to have been recognised as one of the forerunners in developing new models of care to improve the health and healthcare of people with long-term conditions.

“The announcement that we have been chosen as one of 14 Vanguard sites chosen to test the ‘multispecialty community provider organisational model’ identified in NHS England’s Five Year Forward View shows that we are making excellent progress in joining health and social care for the benefit of patients.

“Our aim is to meet the needs of our population by ensuring coordinated services which support much more effective condition management so that patients to stay well for longer.

“This follows the announcement by Care and Support Minister Norman Lamb in January that the Fylde Coast had been chosen to become an Integrated Care Pioneer to support our plans to develop more personalised and coordinated community-based care for older patients with a number of complex long-term conditions and the greatest health needs.

“This achievement is the result of an enormous amount of hard work from all partners involved in developing new models of care – and will really improve the quality of services and the experience of patients and carers.”
